Recovery Enhanced Comprehensive Pain Management

Treatment at REMI begins with a comprehensive evaluation.  A thorough medical history is obtained to determine the source(s) of pain and treatments that have been utilized in the past. Next an analysis of evidence- based factors that can impact successful pain management is done using modern objective screening instruments.  This include assessments of psychosocial wellness and level of behavioral health sensitivity. Input from family, friends and previous providers is sought to help understand  the nature and extent of the chronic pain experience. This evaluation combined with the unique goals of the pain sufferer help create an individualized treatment plan to meet the pain recovery needs that have been identified.  A customized combination of the 3 M's is created.

  • Medical Risk-Reduced Treatments, including pain medications 
  • Movement Therapies  
  • Mind Based Therapies 

Effective Risk-Reduced Medication

Effective pain medications that have a reduced risk compared to traditional opioids are utilized to decrease chronic pain. Non-controlled analgesic such as antiepileptics, antidepressants, muscle relaxants and the risk mitigated opioid buprenorphine are tailored to the type of pain. For those with Opioid Use Disorder and chronic pain, buprenorphine can be used as part of Medication Assisted Treatment.

Movement

A physical movement element is essential to recovery from chronic pain and opioid related disorders. At REMI treatments offered include passive (massage and acupuncture) and active (Tai Chi and related martial arts and yoga) movement therapies. These therapies are ongoing and integrated into a persons life as opposed to traditional time-limited physical therapy treatments.

Mind Based Therapies

Chronic pain and opioid related disorders negatively effect thinking and behavior.  Therefore at REMI a mind based cognitive, emotional and behavioral program is developed with individual and group education and therapy and cognitive remediation strategies tailored to the unique needs of the sufferer.

REMI Chronic Pain Recovery Services



Initial Evaluation


60-90 minutes

An initial evaluation will include objective evidence-based instruments to measure risk factors for ORD, urine drug screening, prescription drug monitoring program (PDMP) surveillance, a history and physical examination, laboratory evaluations as clinically indicated. Neuropsychological testing, family/significant other involvement and physical conditioning assessment will be done as clinically indicated.



Follow up Medical Management

Every 1-6 weeks as indicated


Follow up medical appointments with behavioral health pain specialist MD will utilize risk mitigated analgesia with the evidence-based monitoring tools mentioned above. Follow-up appointments  be done individually or in a group format  as clinically indicated.



Pain Recovery Intensive Outpatient Program

3-9 hours per week


Programming will occur each week involving the following groups:

- Chronic Pain Anonymous group

- Family Education and Support group

- Pain Self-Management Group

- Movement Group (options include Tai Chi or Yoga

- Mindfulness meditation group

- Cognitive Behavioral therapy group

Individual therapy sessions will be arranged for other forms of appropriate psychotherapy, cognitive remediation sessions, massage therapy, acupuncture, and neurofeedback

Encouraged involvement in on-site activities  includiing

- Music therapy

- Recreational activities/outings

- 12 step meetings including Alcoholics Anonymous and Narcotics Anonymous
